What is primary health care and for what reasons should I make an appointment with a primary health care provider?

When should I seek primary health care?

At a primary health care clinic, a patient has the opportunity to book regular appointments with the same healthcare provider, which ensures better consistency and continuity in the care they receive. 

 

A primary health care provider may be consulted for:

  • health screenings

  • vaccinations

  • cold, congestion, fever

  • management of chronic conditions

    • diabetes​

    • heart disease

    • hypertension

    • obesity

    • allergies

    • asthma

  • minor wounds, cuts, stitches

  • sprains, strains

  • health problem where you need advice

  • minor burns

  • earaches 

  • migraines

  • bladder infection

  • checkups 

  • prescription refills

  • reproductive health

Primary health care supports a patient's holistic health needs through health promotion, disease prevention, treatment, rehabilitation, illness and injury prevention, and care coordination.

Primary care can include doctors, nurse practitioners, dieticians, foot care nurses, counsellors, dental hygienists etc.

What services will this primary care clinic offer?

Primary care providers offer a broad range of services that support a patient’s holistic health needs, including preventive care, health promotion, diagnosis, treatment, chronic disease management and family planning.  

​

How frequently will primary care providers be in Kashechewan?  

Mushkegowuk Health primary care providers will be in Kashechewan on a rotational basis. Clinics will be offered two weeks every month.

​

Will my appointments be with the same primary care provider every time?  

Once you become a patient, you will be seen by our Physician, Nurse Practitioner and/or our team of nurses. Click here to see our team members who you will be seeing and talking to to schedule your appointments. 

 

APPOINTMENTS  

How do I become a patient with Mushkegowuk Health?  

Complete the intake package. You can also pick-up an intake package at the reception desk at Kashechewan Health Services. Please complete the form and bring it to Kashechewan Health Services who will fax us a copy of your package. Alternatively, you can email your package to primarycare@mushkegowuk.ca or fax it to 705-268-0435. Once we have received your intake package, we will contact you to schedule an appointment. Our team can also assist you with completing the form - please call us to schedule an appointment to complete your package: 705-269-6662.​

How do I schedule an appointment?  

Please call us at 705-269-6662 or send us an email at moma@mushkegowuk.ca with the subject line "Kashechewan Primary Care."​

What should I bring to my first appointment?  

Bring your health card, and a list of all the medications you are currently taking. Your primary care provider will also ask you about your medical history, including past medical conditions, surgeries and treatments, your current health concerns and symptoms, and any known allergies.  

You can also prepare for your appointment by writing down a list of questions you have for your health care provider, and taking notes about your symptoms.

Where do I go for appointments? 

Please go to Kashechewan Health Services 10 minutes before your scheduled appointment time and check-in with the front desk. 

 

HEALTH CONCERNS  

What should I do if I have a health concern between appointments?  

If you are having a health emergency, please call 705-275-4004 or visit the nursing station immediately. For non-urgent health concerns such as prescription refills, questions about a new medication, you can give us a call at 705-269-6662 to request a phone appointment.

 

You can also contact Health811 by calling 811 or chat online by visiting https://health811.ontario.ca to speak with a registered nurse 24 hours a day, 7 days a week.

CANCELLATIONS  

What if I need to cancel or reschedule my appointment?

If you are unable to make your appointment, we ask that you provide us with 24 hours notice of cancellation. Please call us at 705-269-6662 to let us know and we will cancel your appointment.  We appreciate you letting us know when you can't make it so that we can try to fit in other patients who may be waiting for an appointment. 

 

What happens if Mushkegowuk Health needs to cancel my appointment?  

If we need to cancel your appointment, our team will give you a call to notify you of the cancellation and will rebook you on the next available date.

Please fax or email your completed forms to: 1-888-777-5708 or primarycare@mushkegowuk.ca. You can also drop-off your completed forms to reception at Kashechewan Health Services.

Please remember to bring your Ontario Health Card with you to your appointment. ​ If you do not have an Ontario Health Card and would like assistance obtaining one, please contact our Patient Advocate or Patient Navigator at 705-269-6662.
